Disconnecting Windstream line from my house

Hello All,
I have read some of the posts regarding disconnecting the telephone company line, however I was unable to find a box like the one Windstream has used on my house. Can someone please help me with the best way to disconnect the telephone company line and keep all of the phone jacks in my house connected so I can distribute Ooma to all. I am not using DSL anymore. Please see the attached pictures.
