Poor Voice Quality

Would someone be kind enough to take a look at my stats and provide any advice as to how I can improve the voice quality on my Ooma?

The problem that I'm having is that I'm experiencing frequent breakups during phone conversations.

Those first numbers are terrible. You do have some delay-issues somewhere to resolve. Anything from the way Ooma is wired into the LAN system, to routing / destination concerns.
