Problem: incoming calls suddenly go straight to voicemail

My settings are 45 seconds before voicemail should pick up, but all calls go directly to voicemail without the phone ever ringing.
What's wrong with this?

Check to see if your Envelope (voicemail button) is on. If it is, hold it down for 5 seconds to turn off "privacy mode".
