As I understand your discription, the Telo is contstantly rebooting. And if you can no longer connect to the setup.ooma.com page via the HOME port on the Telo, then your devices is most likely dead.
Note you should be even able to connect to the HOME port setup page even if the Telo was NOT connected to a modem.
