Online forms don't work with Skype number

I am trying to port my Skype phone number (local Skypein) to ooma. No joy.
I use the Port form, and select OTHER for connection type, and OTHER for PROVIDER. I select VOIP or DIGITAL SERVICE.
I cannot get past the "specify provider" on the form, it give the red error message specify provider over and over.
I emailed support, and they did not help, they did not understand what I was trying to do?
I have previous asked phone support can I port Skype, and they said yes (just a question, not a request).
HELP? I want to port the number before it expires.
