Does anyone know if the $4.99 includes the Federal Universal Service Fund (FUSF) fee that ooma usually charges on top of their rates? And if not, are fees based on the $4.99 or the actual number used ?

It looks like it's only available to Premier customers. It's not even available during the free Premier trial as far as I can tell.

I don't know about the availability for free trial Premier, but I'm paid Premier and I see the choice available in my.ooma "Add-Ons".

Yea, I don't have it available in my free trial Premier account, but it is available with my Premier annual subscription account.

I am technically still in my free trial (since my 6 month trial wouldve ended in Jan 2010) but I paid ahead in order to get my phone port so my premiere expires Jan 2011. Its available for me in the add-ons. So I am guessing paying for one year turns my "trial premier" months into a "paid premier". Nice.
