Can't call certain numbers

I've been having a problem with my Telo for several weeks now. It works just fine for most numbers, but there are a few numbers that just don't want to connect. We get something different every time - endless ringing, busy tones, answering machines that belong to someone else, "voicemail has not been set up" messages. This seems to be limited to numbers in the 518-325-XXXX and 518-329-XXXX exchanges. Tried rebooting the Ooma unit and the router, but the issues persist - although I am getting through successfully about half the time today. We had this problem about a year ago, but it resolved on its own after a couple of weeks. Does anyone have any insight into what could be wrong?

Does your online call log show that you actually dialed the number you were trying to dial?
It could be the phone having dialing trouble.

Yes, the number shows up in the call logs correctly, but the call isn't being directed to the correct number. I just dialed a certain number three times, and got three different results.

Are you dialing 11 digits?

or

10 digits if you have 10 digit dialing enabled?

Manual dialing or speed dialing?

When the Telo hears the first touchtone it turns the outbound audio path off and then regenerates the tones that go to the next touchtone receiver in the chain.

Yes to dialing 11 digits - I'm manually dialing 1-518-325-XXXX. The 10-digit dialing setting is not enabled.
